What is open enrollment?
Open enrollment is the time each year when you are able to enroll in a new health plan or change from one plan to another through your health insurance at work. The choice you make determines your insurance coverage for the next year. Therefore, it’s important during open enrollment to choose a health plan that allows you to connect with the doctor and/or hospital you want.

How do I select a Woodland Healthcare Primary Care Physician?
Your Primary Care Physician is your personal physician, and he or she is responsible for coordinating all your health care needs. It is important that you establish a relationship with your Primary Care Physician as soon as possible by making your first appointment within one month of becoming eligible for care. If you have questions, or need assistance in selecting a Primary Care Physician, please call our Physician Finder Line at 530.668.2691.

How do I transfer my medical records to Woodland Healthcare?
As a new patient, there are two options to have your medical records transferred to Woodland Healthcare:

| Woodland Healthcare’s Release of Information (ROI) Department handles the transfer of medical records. You can obtain the forms you need to complete to have your medical records transferred from your new doctor at Woodland Healthcare or by going directly to the ROI office located at 1207 Fairchild Court. Once you complete the form, it must be returned to ROI. ROI will send the request to your previous doctor. Once the records are received, they will be added to your medical record at Woodland Healthcare. If you have any questions, please call ROI at 530.668.2640. |

You can complete paperwork at your former provider’s office and have your records sent to:

Woodland Healthcare does not charge for this service, however, other facilities may. Any patient wishing to have records transferred MUST sign a release form in order to have their records sent to Woodland Healthcare or another medical group or provider.
How do I schedule an appointment with my new Woodland Healthcare Primary Care Physician?
Woodland Healthcare offers appointment-scheduling services through our centralized Call Center at 530.668.2600, Monday through Friday, 7:30 a.m. to 7:30 p.m. and Saturday, 7:30 a.m. to 3:30 p.m. For your convenience, you may schedule, reschedule or cancel an appointment in any of our doctors’ offices with one phone call. Please give more than a 24 hour notice.
What should I bring with me for my first visit?
- All your medications
- Your family medical history
- Names of emergency contacts/telephone numbers
- New Insurance information or card
- Photo identification
