How do I request a copy of my personal medical records?
The procedure for obtaining copies of your personal medical records varies, depending on your specific request (some fees may apply). To request a copy of your personal medical records, please call the Release of Information Department at 530.668.2640.
As a new patient, there are two options to have your medical records transferred to Woodland Healthcare:

| Woodland Healthcare’s Release of Information (ROI) Department handles the transfer of medical records. You can obtain the forms you need to complete to have your medical records transferred from your new doctor at Woodland Healthcare or by going directly to the ROI office located at 1207 Fairchild Court. Once you complete the form, it must be returned to ROI. ROI will send the request to your previous doctor. Once the records are received, they will be added to your medical record at Woodland Healthcare. If you have any questions, please call ROI at 530.668.2640. |
