Yolo Adult Day Health Center is a program specifically designed for adults struggling to function independently. Our diverse program of health, social and rehabilitation services promote the well-being, dignity and self-esteem of an individual. Our goal is to maximize independence, improve management of chronic symptoms, prevent hospitalization and/or premature nursing home placement and provide support and relief to caregivers.
We assist all adult residents of Yolo County who may:
- Have trouble with everyday tasks like bathing, toileting or walking
- Need assistance in managing medication or chronic health conditions such as diabetes or high blood pressure
- Be recovering from stroke, illness or injury
- Be isolated, depressed, confused or suffering memory loss due to disease or a related condition
The Yolo Adult Day Health Center offers trained and caring professionals and paraprofessionals who provide support, enjoyment and stimulation in a safe and comfortable environment. Individual programs are tailored to meet the needs of the participant and those they love, taking into consideration physical and cognitive limitations.
Alzheimer's Day Care Resource Center
The Alzheimer's Day Care Resource Center (ADCRC) is a specially-funded program designed to meet the unique needs of individuals with Alzheimer's Disease or other related dementia. We call this program CASA: Caregiving Assistance, a Safe Approach.

How To Enroll
Just call. We will help you from there. We begin with an initial assessment and then, with input from participant, family and doctor, develop an IPC. Participant programs are reviewed quarterly and adjusted to meet changing needs.
Costs
Medi-Cal and some private insurance will cover the cost. Private pay rates vary. Call for cost information. Our daily fees include services, meals and transportation.
